Have you given out information to someone who may use it for fraud? Do you believe you are a victim of identity theft?

  1. Document all the names and phone numbers of everyone you speak to regarding the incident. Follow up phone calls with letters. Keep copies of all correspondence.

  2. Call your Bank(s) and Credit card issuers immediately.
    Close any accounts that may have been compromised.
    Close any accounts that were opened without your consent.

  3. Contact your local police department to file an Identity Theft Report.

  4. Contact the 3 major credit reporting agencies.
    Request the agencies place a fraud alert and a victim's statement in your file. Request a copy of your credit report to check if any accounts were opened without your consent.
    Request that the agencies remove inquires and fraudulent accounts stemming from the theft.

  5. Contact the Social Security Administration Fraud Hotline to report the unauthorized use of your social security number.

  6. Notify the Dept. of Motor Vehicles of the theft and check to see if any driver's licenses have been issued in your name.

  7. Notify the passport office to be on the lookout for anyone ordering a passport in your name.

  8. File a complaint with the Federal Trade Commission. www.ftc.gov

  9. File a complaint with the Internet Crime Complaint Center.
