2. Print your name and complete address on this form.
3. Select the gift card of your choice.
4. Give this coupon to your friend, family member, or co-worker and recommend any of our fantastic checking accounts.
5. When they open a new checking account with us and give us this coupon, we'll mail you the $15 gift card of your choice.
